Preparing for your First Visit

Thank you for choosing Baton Rouge Urology for your urologic needs.  In preparation for your first visit to our clinic, please see the following information:

Ways to prepare for your appointment

-Ensure your provider and Baton Rouge Urology accepts your insurance.  You may verify this by calling the number on your insurance card and asking your insurance carrier if Baton Rouge Urology is in-network with your plan.

-Arrive on time for your appointment.  Generally, we ask our new patients to arrive 15 minutes prior to their scheduled appointment to allow for the completion of paperwork.

-You may complete your new patient paperwork and bring it with you to your appointment.  The paperwork can be found here. (Link to new patient paperwork)

 

What to bring with you to your appointment

-A physical copy of your insurance card and a photo ID (Driver’s license, passport, Military ID or government issued ID).

-Payment for any copayment, deductible or coinsurance per your insurance requirements.  Our office collects any patient responsibilities at the time of service.  We accept cash, credit or debit cards for your first visit.

-A list of your current medications along with any pertinent medical records that may be necessary (example: if you will be seen for an elevated PSA level, please make sure to either bring the PSA labs or have them sent to our office).

 

What will happen at my appointment?

Once you have been greeted by our reception team, we will verify your information including address, telephone number as well as insurance information.  At most visits, you will be asked to provide a urine specimen. Our physician team will assessyour condition and order any necessary tests.  In some cases, the tests can be performed in our office.
